Our In-home Family Advice and Planning Session brings your family together with a LiveWell Care consultant to unpack your individual situation, the current system and upcoming reforms. This one-off, flat-fee session delivers the clarity, realistic expectations, and expert guidance you need to make informed, independent decisions. Any ongoing support you choose after this session is billed at an hourly rate

 

Stella and Ivan’s story

Stella and Ivan were self-funded retirees in their mid 80’s. Stella had been supporting her husband Ivan at home since he was diagnosed with Parkinson’s disease 8 years ago. Over the previous 12 months, Ivan’s condition had deteriorated resulting in Stella having to provide a higher level of support to him. Stella was finding this increasingly difficult and found that she was experiencing both increased physical and emotional stress as a result of her caring role. LiveWell Care was engaged to help Stella and Ivan navigate the community care system and establish what combination of supports worked best for them. With LiveWell Care’s assistance, Stella and Ivan were able to gain a greater understanding of the level of co-contributions they would be likely to pay if accessing a Support at Home Package. Armed with this information they decided they would access a local Commonwealth Home Support Program provider for domestic and personal care supports and top up these services with privately funded services. LiveWell Care were able to support Stella and Ivan access these services.

A LiveWell Care consultant can act as an independent third party who can be neutral, objective and can support family members of an older person to:

  • quickly comprehend the complexities of the service system
  • facilitate communication between family members
  • support the family through the planning process, as to;
  • develop an agreed care plan to support the older person at home

The Mitchell Family’s story

The Mitchell siblings engaged LiveWell Care to facilitate a family planning session in the home of their mother Sarah, who was accessing respite at a residential facility. The four siblings all had concerns about their mother and wanted the best for her, but they had different levels of understanding regarding the community care sector and what could be provided. Each had varying views about the level of risk they were comfortable with when it came to their mother living alone at home. The family’s LiveWell Care consultant was able to act as an independent third party who could be neutral, objective and support them come to an agreement, which was in the best interest of their mother. LiveWell Care was able to map out the community care sector and provide the families with clear expectations of such services. LiveWell Care was able to provide solutions to alleviate concerns relating to particular risks and also discuss the concept of ‘dignity of risk’ and that not all risk could be eliminated. The planning session lasted 2 hours and the family were able to come to an agreement and develop a plan to support their mother to remain living at home.
