The LiveWell Care Team is made up of dedicated, passionate and professional Care Consultants with years of experience in dealing with the issues and challenges associated with the provision of aged care services.

Your Care Consultant will work with you to develop a Care Plan that takes into consideration all aspects of your life. They will assist with implementing the Care Plan and can also manage care service providers on your behalf.

LiveWell Care’s independent and personalised aged care consultancy services can be broken down into the following stages:

1. Initial consultation and discussion of funding options

The primary purpose of this initial consultation is to provide an opportunity for a LiveWell Care Consultant to clearly explain the current government funded options and processes. It also allows the Consultant to explain the benefits of using LiveWell Care. At the end of this consultation you will receive an information pack including an up-to-date directory for relevant community supports and services in your area.

2. Assessment and LiveWell Care Plan development

A LiveWell Care Consultant will visit you in your home and undertake an in-depth assessment to gain an understanding of you, your lifestyle and your current and future needs.Your LiveWell Care Consultant will then be able to match your needs to the most appropriate service response (government funded, private or a combination of both) and will develop a personalised Care Plan.

The resulting LiveWell Care Plan will outline the service tasks, type and quantity of services required in addition to providing details of recommended care service providers in your local area that suit your specific needs.

3. Implementation of your LiveWell Care Plan with your choice of recommended high quality care service providers

Once your Care Plan has been presented to you, discussed and agreed upon, your LiveWell Care Consultant will implement the Care Plan on your behalf. This will include the coordination of the various elements of the plan with the selected recommended care service providers.

Your Care Consultant will be responsible for ensuring that your needs identified during your in-home assessment are clearly articulated to providers and that your relationship with these care providers is set up to run efficiently and cost effectively.

They will also ensure that services are set up and implemented in line with your choices and preferences; maximising your independence and maintaining your quality of life.

4. Management of your Care Plan with ongoing reviews as required

For complete peace of mind your dedicated LiveWell Care Consultant will take on the management of your Care Plan. This will help ensure that you experience continuity of care, security in the guaranteed provision of service and the highest standard of care from your providers.

The management of a Care Plan is a dynamic process. Regular monitoring and reviews conducted by your LiveWell Care Consultant will ensure that the necessary level of care is provided as your needs change and evolve.

This service incurs a weekly fee and has a minimum engagement period of three months.

Mabel had been living independently in her own home, but after a receiving a diagnosis of dementia, and experiencing a few falls, her family were concerned about how much longer she would be able to remain living at home. LiveWell Care were appointed to assess Mabel’s needs and support the family in understanding what levels of support could be provided and what combination would best meet her needs. Working with Mabel and her family, appropriate services based on her lifestyle requirements were identified. A combination of funded services from her local council and non-funded services from private providers were then engaged. Given the progressive nature of Mabel’s dementia the family engaged LiveWell Care to provide ongoing case management to monitor and review Mabel’s care needs and adjust her care plan accordingly over time. Mabel was able to continue living in her own home and participate in the decision-making regarding her care. Mabel and her family were able to use their LiveWell Care consultant as a sounding board to make informed decisions and ultimately have peace of mind that they were exploring all their options.
