You need to advise your current provider that you intend to leave and agree to a cessation date. You also need to contact My Age Care to advise that you want to change providers and ask them to provide you with new referral code that you can use to approach new providers. Once you have chosen a new provider and agreed on a commencement date the new provider can negotiate with the old provider to have any remaining packaged funds transferred to them.
