- quickly comprehend the complexities of the service system
- facilitate communication between family members
- support the family through the planning process, as to;
- develop an agreed care plan to support the older person at home
Supervised Visits
A LiveWell Care consultant can provide supervised visits between an older person and other individuals in their home or place of residence.
Welfare Checks
A LiveWell Care consultant can provide welfare checks on an older people residing in residential care care or at home where family and loved ones are unable to do so due to distance.

The Mitchell siblings engaged LiveWell Care to facilitate a family planning session in the home of their mother Sarah, who was accessing respite at a residential facility. The four siblings all had concerns about their mother and wanted the best for her, but they had different levels of understanding regarding the community care sector and what could be provided. Each had varying views about the level of risk they were comfortable with when it came to their mother living alone at home. The family’s LiveWell Care consultant was able to act as an independent third party who could be neutral, objective and support them come to an agreement, which was in the best interest of their mother. LiveWell Care was able to map out the community care sector and provide the families with clear expectations of such services. LiveWell Care was able to provide solutions to alleviate concerns relating to particular risks and also discuss the concept of ‘dignity of risk’ and that not all risk could be eliminated. The planning session lasted 2 hours and the family were able to come to an agreement and develop a plan to support their mother to remain living at home.
